INTRODUCTION

California Support Services is dedicated to empowering California communities through comprehensive wraparound services. From educational programs to mental health support, we believe in providing the tools and resources necessary to help individuals and families thrive.

Key Statistics/Impact Metrics:

  • Served over 7,322 individuals in Contra Costa County within the past 10 years.
  • Supports 2,585 youth, young adults & adults annually through mentorship, preventive services and educational programs.
  • Assisting those experiencing homelessness in finding secure, supportive housing for over 3 years.
  • 70% reduction in recidivism helping formerly incarcerated individuals reintegrate and succeed.

Enhanced Care Management

Enhanced Care Management is a Medi-Cal benefit designed for members with complex medical and social needs. Our ECM services provide a dedicated care team to coordinate comprehensive care, ensuring that members receive the support they need to achieve better health outcomes.

Housing Transition Navigation Services

  • Tenant screening and housing assessments​
  • Development of individualized housing support plans​
  • Housing search and application assistance​
  • Securing necessary documentation and benefits advocacy​

Housing Deposits (financial assistance)

  • Security deposits and utility setup fees
  • First and last month's rent
  • Essential household items to ensure a safe living environment​

Housing Tenancy and Sustaining Services

  • Landlord coordination and dispute resolution​
  • Assistance with annual housing recertification​
  • Linkage to community resources to prevent eviction
